Professional Disclaimer and Scope of Service
Our offerings are not intended to serve as a substitute for diagnosis, treatment, or advice from a Licensed Medical or Healthcare Professional.
This includes, but is not limited to, Physical Therapy, Chiropractic, or Acupuncture. Clients with serious or chronic health concerns are required to seek clearance from a qualified healthcare provider before booking a session.
Client Responsibility and Acceptance
Full disclosure of all pre-existing health conditions is mandatory for every client prior to treatment.
By electronically signing our waiver and/or submitting your intake form, you hereby acknowledge and affirm that you have read, understand, and agreed to all terms and conditions of our services.
Scope of Treatment
All sessions include any combination of modalities and techniques such as assisted stretching, corrective exercises, cupping, and percussive techniques, which constitute the MOBILITI method. The specific techniques and duration of their use are at the sole discretion of MOBILITI and its practitioners/agents.
Right of Refusal and Liability
We reserve the right to refuse service to anyone for any reason at any time. Furthermore, MOBILITI and its agents are not held liable for any punitive damages done to your person or property.
Punctuality Policy
We strive for excellence and punctuality is important to us. Due to the unpredictable nature of travel (including weather and traffic conditions), please be advised that we will make every reasonable effort to arrive prior to your scheduled appointment time.
Refund and Cancellation Policy
Service Delivery and Usage
All services rendered are non-refundable. Once a session, treatment, or consultation has been completed, you are responsible for the full payment of that service, and no refunds will be issued.
Prepaid Purchases
Refunds for prepaid packages or services that have not yet been rendered are allowable only within 30 days of the initial purchase date.
Funds will be automatically applied to your original method of payment.
Please allow up to 72 hours (or 3 business days) for your refund request to be processed.
If you haven’t received a refund yet, please contact your bank directly.
Rescheduling and Cancellations
We understand that unexpected conflicts arise.
If you are unable to attend a scheduled session, you must notify us at least 24 hours in advance.
Prepaid sessions cancelled within the required notice period may be rescheduled at no additional charge.
If a reasonable accommodation cannot be made to reschedule, a refund may be issued, provided it falls within the 30-day refund window.
